    Managing clozapine in an acute hospital
    The NCEPOD Treat as One A National Confidential Enquiry into Patient Outcome and Deaths (NCEPOD) Report highlighted: • Only 72% of patients had their mental health drugs prescribed during the initial assessment. • Discharge notification lacked the mental health medication in 29% of patients. • Clozapine, most commonly used in treatment resistant schizophrenia, is considered a critical medicine. Clozapine requires careful initiation, must be re-titrated if doses omitted for more than 48 hours and has several serious adverse effects which must be monitored. Mental Health In-Reach has been operating for two years ‘Bridging the gap between mental and physical healthcare in general hospitals’ with a mental health pharmacist supporting the acute hospital pharmacy team. This has improved acute staff confidence is managing clozapine and enabled effective interventions to improve patient care.

As one of England's largest ICBs with 8 CCGs amalgamating in 2022, we harnessed opportunities to work at-scale, making best use of digital technology and retaining links with frontline services providing patient care.     Stopping racism at the front line of pharmacy
    PDA members see the impact of racism at the frontline. In this panel event, members of the PDA BAME (Black, Asian and Minority Ethnic) Network will discuss key trends in racism in pharmacy in relation to available data such as the NHS Workforce Race Equality Standard (WRES). They will then speak about how and why employers need to challenge and address racism wherever it is present and commit to having anti-racist workplaces. Attendees will learn how resources such as the PDA Anti-Racist Pharmacy Toolkit are critical to ensure that racism can be driven out of their working environment.

The project included two phases, phase 1 included collecting responses to a research engagement questionnaire, and phase 2 included collecting further views through individual interviews and/or a focus group with pharmacy professionals.     We will present our learning from the innovative pilot study undertaken to develop and investigate a pilot prescribing training program for trainee pharmacist in their foundation year. Insights to successful strategies for gathering portfolio evidence and how to identify appropriate activities and assessment tools for prescriber competence will be shared.     In 2024 HIV is a manageable condition, and people living with HIV are living longer, and will be encountered in health care settings outside of the HIV speciality. It is, therefore, important as a generalist or other specialist to have a grasp of how to identify and manage HIV drug-drug interactions. Additionally, due to effective treatment leading to fewer HIV-related deaths & improved survival, as well as ongoing diagnoses, there is an ageing cohort with potentially complex polypharmacy.

    As part of a leadership fellowship project on medicines sustainability, a survey and interviews were carried out with front line pharmacy staff exploring their opinions on environmental sustainability and the barriers and enablers found in practice. We will discuss the findings and how we can use this to move environmental sustainability work in pharmacy forwards.

We're excited to announce that the extraordinary explorer, Ann Daniels, will be joining us as a guest speaker on Saturday 2 November.

Ann’s session will provide powerful lessons on leadership, teamwork, and overcoming adversity, directly applicable to clinical pharmacy. Learn how to navigate challenging situations, inspire your team, and drive innovation in your practice. 

Ann Daniels is record-breaking polar explorer. A former bank employee, with little experience of outdoor pursuits, she worked to be selected from over 200 female applicants to undertake her first assault on the North Pole 20 years ago. Since then she has become one of Britain’s leading woman explorers with a wealth of lessons, insights and experiences in leadership, teamwork and self-belief.
