Discharge Medicines Service - an integrated approach

The Discharge Medicines Service (DMS) became an essential Community Pharmacy service in February 2021. This session will give an insight into the development and continued management of the innovative work of the DMS and Overprescribing Team at The Shrewsbury and Telford NHS Trust. Our aim is provide support to patients with their medication post-discharge, and reduce hospital readmissions. We will discuss how we have formed our team, established our roles, plus the steps we have taken to expand the DMS within the Shropshire, Telford and Wrekin Integrated Care System (ICS). This will include the challenges we have encountered and how we have overcome these with the help of our colleagues within the ICS.

This session will provide an overview of the innovative projects we are working towards, along with the ICS. This includes our approach to reduce the overprescribing of opioids, by improving the quality of discharge summaries, plus creating and providing opioid patient safety leaflets. We will explain how we are expanding our service, enabling us to refer patients that we have identified would benefit from a Structured Medication Review to their Primary Care Networks upon their hospital discharge.
